Why does jaw bone disappear after tooth loss?

Natural tooth roots stimulate the jaw bone every time you bite. When a tooth is removed, that stimulation stops and the bone begins to resorb — shrinking in both height and width. What to expect: before, during, and after

Before surgery — a 3D cone beam CT scan maps exactly how much bone is present and where the graft needs to go. You'll receive written preparation instructions (dietary restrictions, medication adjustments if applicable).

During surgery — local anesthesia is administered and IV sedation is available if preferred. The graft site is accessed, the graft material is placed and compressed, a membrane is typically secured over it, and the site is sutured closed. Most grafts take 30–90 minutes depending on complexity.

Immediately after — swelling peaks at 48–72 hours. Soft foods, gentle rinsing with prescribed chlorhexidine, and avoiding pressure on the site are important for the first two weeks.

Healing phase (3–6 months) — the graft material is gradually replaced by your own living bone in a process called osseointegration. Dr. Gjelaj monitors healing with periodic X-rays and a follow-up CBCT scan before scheduling implant placement.

Implant placement — once the graft is confirmed healed and dense, implants are placed into the new bone. From there, the standard implant timeline applies.

Who is a candidate for bone grafting?

Good candidates are patients who:

  • Had a tooth extracted and want to protect the site for a future implant
  • Have been told they "don't have enough bone" for implants
  • Have worn dentures for several years and want to transition to fixed teeth
  • Have experienced bone loss from periodontal (gum) disease
  • Need a sinus lift because the upper jaw bone is too thin

Factors that affect candidacy include smoking (significantly impairs healing), uncontrolled diabetes, certain medications (bisphosphonates, immunosuppressants), and active gum infection.
