Quick answer: the implant procedure itself shouldn't hurt at all — it's performed under full local anesthesia, with oral or IV sedation available. Afterward, most patients describe soreness milder than a tooth extraction, peaking around days 2–3 and managed with over-the-counter ibuprofen. In our patient surveys, over 85% of All-on-4 patients report pain of 3/10 or lower after the first 72 hours. The anticipation is almost always worse than the experience.

Here's the honest, hour-by-hour version — because "don't worry, it won't hurt" isn't an answer, and fear of pain is the number-one reason people live with missing teeth for years.

During the Procedure: What You Actually Feel

Nothing sharp. The surgical site is fully numbed with local anesthetic before anything begins — you'll feel pressure and vibration, similar to having a filling done, but not pain. Interestingly, implant placement often involves less discomfort than the extraction that may precede it, because bone itself has few pain-sensing nerves; most post-surgical soreness comes from the gum tissue, and modern 3D-guided placement keeps those openings small and precise.

If dental anxiety is the real obstacle, that's solved here too: Trinity Dental Arts offers oral sedation (you're relaxed and drowsy) and IV sedation with a board-certified anesthesiologist on staff (most patients remember nothing). Details on the sedation dentistry page.

Your Recovery Timeline, Day by Day

  • Day 0 (surgery day): numbness fades over a few hours; you'll feel dull soreness, not sharp pain. Rest, ice in intervals, soft foods. Most patients need only ibuprofen.
  • Days 1–3: the peak. Expect swelling, possible minor bruising, and soreness comparable to a hard workout in your jaw. Ice, head elevation, and over-the-counter anti-inflammatories handle it for the large majority of patients.
  • Days 3–5: the turn. Swelling recedes, most patients are back to work and normal routines — desk workers often return in 1–2 days.
  • Days 7–10: the site feels close to normal; any sutures are dissolving or removed.
  • Months 1–6 (osseointegration): the implant fuses with your bone. This phase is completely painless — you won't feel it happening at all.

Single implants sit at the mild end of this timeline. Full-arch All-on-4 cases involve more sites and more swelling, yet still surprise patients: over 85% report pain at or below 3/10 after 72 hours, largely because the angled technique avoids grafting in most cases and everything is completed in one precisely planned surgery.

What We Do Differently to Keep It Comfortable

Pain after oral surgery mostly reflects trauma to tissue — so the less invasive the surgery, the easier the recovery. At Trinity Dental Arts that means 3D CBCT planning and guided placement (smaller, exact openings instead of exploratory ones), PRF regenerative therapy made from your own blood to speed healing and reduce dry-socket risk (how it works), an in-house lab that eliminates extra appointments, and a surgeon — board-certified implantologist Dr. Gjelaj — with more than 10,000 implants placed. Experience is the best anesthetic there is.

When Soreness Isn't Normal

Mild soreness fading over a week is expected. Call us — (727) 228-6846 — if you notice pain that increases after day 3, throbbing that ibuprofen doesn't touch, fever, or a bad taste from the site. These are rare, they're treatable, and catching them early keeps them trivial. You'll leave with written aftercare instructions and a direct line.

Frequently Asked Questions

Does getting a dental implant hurt more than a tooth extraction?

Almost always less. Patients who've had both consistently rate implant placement as the easier experience — the site is smaller, the technique is more precise, and bone has fewer pain receptors than an inflamed tooth.

How long will I need off work?

Desk job: usually 1–2 days. Physical work: 3–5 days to avoid raising blood pressure at the surgical site. Full-arch patients typically take 3–5 days.

What pain medication will I need?

Most patients do well with over-the-counter ibuprofen, sometimes alternated with acetaminophen for the first 48–72 hours. Prescription analgesics are available for bigger cases but frequently go unused.

Does a bone graft make recovery worse?

Only modestly — socket grafts add little discomfort. And many patients who fear they'll need bone grafting turn out not to: All-on-4's angled placement avoids grafting in most full-arch cases.

I'm terrified of dental work. Is sedation really an option?

Yes, and you're the exact patient it exists for. IV sedation with our board-certified anesthesiologist means you're monitored the entire time and remember essentially nothing. Fear shouldn't cost you your teeth — read how sedation works here.
